Credit Card Authorization

A credit card on file is required to book an appointment for our no-show/cancellation policy. Your appointment will not be secured until we receive the credit card information.

This is NOT a payment.

Tentative appointments auto-delete after 24 hours without the credit card information,

and will be open to other inquiries.

I understand and agree that this card will be charged $50 should I no-show the appointment or cancel the appointment with less than 24-hours notice prior to the appointment start time. I understand and agree that this card will be charged for $50 should I arrive to the appointment and then for any reason choose not to proceed with the appointment. These same terms apply if I choose to make additional appointments at Blush after my initial appointment.

I authorize Blush Bridal Couture to charge this card for $50 if any of the above terms are violated.
